Nursing Experience in a Hospital without Guardians

Abstract
This phenomenological study used in-depth interviews to clarify nurses¡¯ subjective experiences of caring for inpatients in wards without guardians. Methods: The participants were 10 nurses with more than 3 months of experience in caring for inpatients in wards without guardians in ¡°A¡± General Hospital, which has been in pilot operation as a hospital without guardians for more than 1 year. Results: Five themes and 14 sub-themes were derived. The 5 themes were ¡°Starting with anticipation and pressure,¡± ¡°Fear of identity confusion from changed and increased work,¡± ¡°The increasing dependence of patients and lowered self-esteem of nurses,¡± ¡°Confidence from expansion of professionalism,¡± and ¡°New understanding of a ward without guardians.¡± Conclusions: Professional nursing care service provided by nurses is a good system that should be supplemented and continued. Both short- and long-term studies as well as supplementation of marketing, education, and nursing support are needed to establish the foundation for resolving the cycle of exhaustion in nurses due to excessive workload, proper perception changes in patients and guardians, and the establishment of solutions based on an underlying objective.
